HOW
DO I...
OBTAIN
TELEPHONE ADVICE?
Patients should note that a telephone
consultation is intended to be brief and in certain circumstances is no substitute
for attending the practice either by appointment or during open surgery.
Telephone
calls requested by patients who need to speak to a doctor will be returned between
10.00 - 11.00am.
During the afternoon patients wishing to speak with a
doctor can do so by telephoning the practice between 3.00 - 3.30pm only.
Please
note that the practice will not telephone patients on mobile telephones. Patients
using mobile telephones wishing to speak to a doctor can do so by calling between
3.00 and 3.30pm and speaking to the duty doctor.
HOW
DO I...
REGISTER?
If you are new to the area and you would
like to register as a patient please call at reception and bring with you your
medical card. All new patients will be invited to attend for a brief health assessment.
Temporary
Residents
Holidaymakers and anyone staying or working temporarily
in the area and in need of medical attention or advice are welcome to make an
appointment to see the doctor or nurse. Repeat medication and contraceptive services
can also be provided.
NB: It is not essential to have your medical card
with you, though it is helpful if it is available.
HOW
DO I...
OBTAIN
HOME VISITS?
It is the policy within the practice
to request that patients attend open surgery or speak with the doctor before a
home visit request can be accommodated. Home visits are for those who are housebound,
very elderly and the seriously ill. If you require a visit, where possible, please
phone before 10.30am on the day the visit is required. The receptionist will
request details of the patient, and the problem, so that the doctor can assess
the priority of the call. We provide emergency cover at all times and calls requested
after 10.30am will be dealt with by the duty doctor on call. Lack of transport
would not normally be considered an indication for a home visit. It is not
usually harmful for a child with a fever or common childhood illness to come to
the health centre.
HOW
DO I...
CHANGE
MY DETAILS?
Please inform reception at earliest
opportunity.
HOW
DO I...
OBTAIN
REPEAT PRESCRIPTIONS?
Repeat prescriptions can be ordered
by letter, telephone or in person to reception. Please include an SAE if you wish
your prescription to be posted. 24 hours’ notice is required for repeat
prescriptions. Repeat prescriptions will be issued no earlier than two weeks before
the date on which they are due.
HOW
DO I...
OBTAIN
SICKNESS CERTIFICATES?
If you are absent from work due
to illness for a period of up to six days, and your employer requires a certificate,
you should complete a self-certification form (available at reception). If you
are ill for more than six days, you must see your doctor to obtain a medical certificate.
Charges
And Fees
Certain services provided by your doctor, eg signing
passport applications, private medical certificates, insurance claim forms etc,
are not covered by the NHS and you may be asked to pay a fee. Our receptionist
can advise about the various fees.
